Healthcare Services facility, an individual's home, a hospital, the jail,
or at one of our Qualified Provider Community (QPC) member sites.

All new clients will enter the Area Authority through Access, Screening, Triage & Referral located at 165 Center Street, Jacksonville in Onslow County and 3820 Bridge Street, Morehead City in Carteret County and will be screened in order to assess the therapeutic needs of the consumer.
Directions
to 215 Memorial Drive:
From Highway 17 or Highway 24
Turn on to Western Boulevard. Memorial Drive intersects with Western Boulevard at the traffic light across from the Jacksonville Mall. Facility is on Memorial Drive in the wooded area behind Onslow Memorial Hospital.
Directions to 3820 Bridges Street:
From Hwy 24 going into Morehead City bear right onto Arendall Street (US-70 E). Turn Left at North 35th Street, then turn Right onto Bridges Street.
Directions to 165 Center Street:
From Highway 17 or Highway 24
Turn on to Western Boulevard. Center Street intersects with Western Boulevard at traffic light across from Grand China Buffet. Facility is a two story brick building with blue roof.
