Any OCBHS forms or documents will be located in the documents section. The entire document library can be searched by using a search keyword or phrase. Providers shall ensure compliance with the MOA for Medicaid Services and the State Contract that identifies the responsibilities and obligations regarding Health Insurance Portability and Accountability Act (HIPAA) and Confidentiality. |

In an effort to reduce delays associated with the approval process for CAP-MR/DD MR2 packets, a checklist has been developed for the submission process (can be found in the forms category of the documents section). This checklist MUST accompany the original MR2 form and supporting documentation (psychological evaluation which includes an adaptive behavioral assessment) being submitted for processing. It will be the responsibility of the consumer’s Case Manager or Provider Agency to ensure all items on the checklist are complete before submitting the packet. *MR2 packets should be submitted to OCBHS, att: Joyce Mozell, 165 Center Street, Jacksonville, NC 28546. |
