CAP PROVIDER ASSISTANCE

In an effort to reduce delays associated with the approval process for CAP-MR/DD MR2 packets, a checklist has been developed for the submission process.

This checklist MUST accompany the original MR2 form and supporting documentation (psychological evaluation which includes an adaptive behavioral assessment) being submitted for processing.

It will be the responsibility of the consumer’s Case Manager or Provider Agency to ensure all items on the checklist are complete before submitting the packet.

*MR2 packets should be submitted to OCBHS,

att:  Joyce Mozell, 165 Center Street, Jacksonville, NC  28546.

PROVIDER ENDORSEMENT

Providers can access Provider Endorsement documents (Communication Bulletin #44 and #47 and Announcement dated December 15, 2005) on the NC DMH/DD/SAS website.

Due to the approval of the new service definitions, providers must be endorsed for Phase I and II services prior to March 20, 2006 in order to receive reimbursement through Medicaid for those services. We will work with providers to expedite the process and will process applications as they are received. For those providers whose corporate office is outside of Onslow or Carteret County, please submit verification of your corporate endorsement approval from the appropriate LME when you are submitting your application request for site/service endorsement for Onslow and/or Carteret County. Providers with corporate offices in Onslow or Carteret county must apply to OCBHS for corporate and site/service endorsement.  It is imperative that applications be submitted as soon as possible to assure timelines are met. Please submit questions via email.
