Medicaid Part D Guide for Individuals with Disabilities

Advancing Independence: Modernizing Medicare and Medicaid (AIMMM) has released "Understanding Changes in Prescription Drug Coverage for People with Disabilities on Medicare," a guide developed for people with disabilities who receive Medicare, friends and family members, benefits counselors, disability organizations, and others who will assist individuals in taking advantage of the new Medicare drug coverage option. The guide seeks to enable Medicare beneficiaries who receive Medicare benefits to better understand the changes in their prescription drug coverage and what key steps they can take to ensure their prescription drug needs are met. The guide focuses on issues of particular importance to people who are dually eligible.

Formulary Finder

The Centers for Medicare and Medicaid's website now contains an online tool which assists people to find plans in their state that match their required drug list. The online tool; called a formulary finder, can be found at the website listed below and can assist with selecting a prescription drug plan under Medicare Part D that fits individual needs.

I live at the Shepard House.  I have asked my provider to help me write this letter regarding my present case manager, Julie.  Miss Julie informed us several weeks ago that she will no longer be my case manager due to current changes being made at Mental Health.  I feel very sad over this because Miss Julie has been the best case manager I have ever had.  She treated me so well each and every time I ever saw her or spoke with her on the phone.  She always kept her word and returned my calls.  She always set firm, but reasonable limits that I felt were achievable when I acted out or did something stupid.  She believed in me and continued to encourage and motivate me when everyone else had given up on me, (even my family) and the most important thing that Miss Julie did, was make me feel like I was the most important, special person in the world every time she talked to me.  I have achieved things under her guidance and support that I only dreamed of before.  I will miss her so much.  Miss Julie has reassured me that my mew case manager is terrific, so if Julie says she’s ok, then she’s going to be ok! And I don’t feel afraid of being with out her (Miss Julie) while she has to do her new job.

I am in a group home that I love because Miss Julie believed that I could make some necessary changes before I could live in a group home again.  I have made several changes in my behavior and attitude no one else thought I could do it! But miss Julie did!  I feel surrounded by love in my home and a sense of security I have never felt before.  I want to thank Miss Julie from my heart for all the wonderful things she has done for me.


I am a person receiving services through OCBHS.  Through the years I have received many services through OCBHS.  I was the most thankful for their services when I became ill and was in the hospital for several months.  With the services I received I was able to return to living on my own in my own apartment.  I have several providers who assist me with daily living.  I am very happy to be on my own.  I get a lot of help from my providers.  I am thankful to OCBHS and my counselor for their help.

Since my arrival at Harris Home, I have improved in lots of areas including responsibilities such as: probation paid off in the 1st month here, returning to school moving 3 reading levels up, taking “Anger Management” for a short time, but not taking it now Mr. Harris still helps me with that problem.  I’ve been able to express my self and discuss with Mr. Harris about anything such as: my expectations in life, careers, girls, fashions that are in and general events.  Mr. Harris supports me though I still have some problems Mr. Harris continues to be there to help and guide me.

            If I had my choice, I wouldn’t move but I know I’m high functioning and capable of moving out on my own, with Mr. & Mrs. Harris guidance and support I can and will but until then I’m happy and safe.

             I’ve known my caseworker for 3 years, she encourages me, she prompts me and tries to motivate and give me direction.   My care worker is a very caring individual.  If I do something wrong she doesn’t get mad at me, just disappointed, she knows what I can do and it’s better.  I can discuss anything and express myself freely and feel comfortable in doing so.  On a scale 1-10, ten being the best Mr. & Mrs. Harris and my case manager are a 10.
